Personally Identifiable Information

Effective Resources, Inc. considers the information submitted by participating companies to be extremely confidential and does not, in any manner, divulge the identity of individual responses.  Effective Resources, Inc., along with the sponsoring organizations, consider this survey to be confidential.  By receipt of access to this report, the recipient agrees to maintain the confidentiality of its contents, to use the information only for its own company, and further agrees not to distribute the information or the survey outside of the recipient’s company.

Printing of data for use within your company is permitted.  Other reproduction and distribution, in any form, including copying machines, in whole or part, without expressed written permission is prohibited.
